I-20: Sclerotherapy in Recurrent Endometrioma

author

Abstract:

Endometriosis as a common hormone-dependent gynecologic disease is seen in 10-15% of women of reproductive age. On the other hand, 20-50% of women with infertility have endometriosis and 30-50% women with endometrioma are infertile. One of severe form of endometriosis is endometrioma. Surgery is considered as essential treatment for endometrioma with the recurrence rate of 10%. Many researches showed that the second surgery which performed on these patients my lead to premature ovarian failure (POF) or decreased ovarian reserve. Another option for treatment of endometrioma is sclerotherapy using several materials such as ethanol 50-800, methotrexate, tetracycline and anastrazole. Most of studies in this field used ethanol in two methods: 1. Aspiration of endometrioma, irrigation, injection of ethanol and repeated aspiration after 10 minutes. 2. Retention of ethanol after irrigation. It seems that retention is more effective. In a clinical trial study, we investigated ethanol for the treatment of recurrent endometrioma before IVF. The result showed that pregnancy rate was insignificantly increased in patients treated with sclerotherapy compared to control group (33.3% vs. 15%, p>0.05). The results pointed to a better trend toward the ethanol. We think that in the future sclerotherapy may be used instead of repeated surgery more than before.

I-21: Management of Endometrioma

Endometrioma is defined as an ovarian pseudocyst arising from growth of ectopic endometrial tissue, which progressively invaginates the ovarian cortex . Nearly17-44% of women with endometriosis also have endometriomas. Whereas detection of peritoneal endometriosis and adhesions typically requires laparoscopic assessment of the pelvis, endometriomas can be reliably diagnosed by transvaginal ultr...

Assessing the efficacy of aspiration and ethanol injection in recurrent endometrioma before IVF cycle: A randomized clinical trial

BACKGROUND Endometriosis is a common hormone-dependent gynecologic disease with a high recurrence. Laparotomy or laparoscopy is the standard surgery for the large endometrioma. Also, sclerotherapy is basically used to treat different diseases one of which is endometrioma. OBJECTIVE The study was designed to assess the value of transvaginal ultrasound-guided ethanol sclerotherapy in patients w...

Preoperative risk factors in recurrent endometrioma after primary conservative surgery

OBJECTIVE Endometriosis is a common gynecological disorder caused by ectopic implantation of endometrial glandular and stromal cells outside the uterine cavity. Among several types of endometriosis, endometrioma is the only subtype that could be determined preoperatively using pelvic ultrasonography, and guidelines recommend pathologic confirmation of endometrioma greater than 3 cm in diameter....

Recurrent Chest Pain, as a Presenting Sign of Ovarian Endometrioma

Chest pain is a rare sign of thoracal endometriosis associated with endometrioma of the tubo-ovarian endometrioma. We report the case periodic episodes of chest pain concurrent with menstruation in a 35-year-old female, in which ovarian endometrioma was diagnosed and left-sided oophorectomy was performed.
